These Terms of Service ("Terms") govern your use of ethoswellnesspharmacy.com (the "Site") and your communications with Ethos Wellness Pharmacy, a business of Grey Dog IV, Inc. ("Ethos," "we," "us," or "our"). Ethovia Health is a separate, affiliated brand. By using the Site or opting in to our text messaging program, you agree to these Terms.
Use of the Site
You agree to use the Site only for lawful purposes and in a manner that does not infringe the rights of, or restrict or inhibit the use of, the Site by any third party.
SMS / Text Messaging Program Terms
By providing your mobile phone number and opting in (for example, by submitting a form on our Site), you agree to receive text (SMS) messages from Ethos Wellness Pharmacy. The following terms apply to our messaging program:
- Program description: Messages may include appointment reminders, patient care and account notifications, follow-ups, and information about our products, services, and offers.
- Message frequency: Message frequency varies based on your interactions with us.
- Cost: Message and data rates may apply. Such charges are billed by and payable to your mobile service provider. We do not charge for the messages themselves.
- Opt-out: You can cancel the SMS service at any time by replying STOP to any message. After you send STOP, we will send a confirmation message and then stop sending you messages. You may receive additional messages if you opt in again.
- Help: Reply HELP for assistance, or contact us at 305-982-8727 or info@ethoswellnesspharmacy.com.
- Supported carriers: Carriers are not liable for delayed or undelivered messages.
- Consent: Consent to receive automated text messages is not a condition of purchasing any goods or services.
Your mobile opt-in is private. No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties. See our Privacy Policy for details.
